The Position
Portland Fire & Rescue (PF&R) is hiring a Community Health Supervisor for the Community Health Assess & Treat Program!
About the position: Job Appointment: Full time, Limited Duration.
Persons appointed to limited term positions will be designated as limited duration employees whose appointment may conclude June 30, 2025, except for the extension of grants or funding.
Work Schedule: Employees in this position will work 40 hours per week with flexible scheduling options. Shifts include days and evenings, the ability to work any shift and on any day of the week is essential. During training, your shift may change depending on the availability of the trainers. After completing probation, you will be able to bid on an open shift based on your seniority. This position is considered an essential employee and is required to report to duty during inclement weather.

About Community Health Assess and Treat (CHAT): The CHAT program provides first response to low-acuity 911 medical calls in lieu of advanced life support apparatus within the City of Portland. The goal of CHAT is to assess and treat patients in community, provide care coordination to vulnerable and socially isolated populations and follow up within 24 hours of the 911 call.

This program is expected to: - Reduce the number of individuals transported to the emergency department for low-acuity medical related issues that are more effectively addressed in a pre-hospital care setting.

- Improve access to care and resources within the community.
Position Summary Under the general direction of the Program Manager, the Community Health Supervisor is responsible for the day-to-day supervision and support of the CHAT teams. This specific position will support the aftercare service line which includes the CHAT Follow-Up Team. This position also supports Medical Responders working in the field. Responsibilities include supervising the workload of assigned units; assigning work to subordinate staff; overseeing staff activities to ensure the achievement of department goals and objectives; coordinating and supervising department programs and functions.
The Community Health Supervisor will also facilitate internal communication between staff and management; interface with outside agencies; monitor system status; facilitate short-term scheduling needs, equipment and supply maintenance; and provide operational support functions as assigned by the manager. Reports to the CHAT Program Manager.

As a Community Health Supervisor, you will: - Lead, supervise, and provide assistance to CHAT units working in the field.

- Direct, monitor, communicate with, and evaluate all employees in their assigned area.
- Ensure maintenance of quality operations through daily contact, oversight, communication, and evaluation.
- Liaison with clients, public safety agencies, community-based organizations, hospital personnel, and other stakeholders.
- Ensure reports and paperwork are reconciled daily; prepare reports and manage projects as assigned by the Manager.
- Ensure adequate levels of equipment are available for each unit.
- Enforce and adhere to established policies and standards.
- Maintain communication with other agencies and departments; serve on committees with other departments to recommend policies and generate ideas.
- Provide professional assessment, consultation, and coordination.
- Perform other related duties as required.
- Physical demands :
- May move equipment or people weighing up to 200 lbs.
- Bend, stoop, kneel, squat, and stand on uneven surfaces for extended periods.
- Using a computer device on a moving vehicle.

To Qualify
Required Licenses/Experience:
- A valid state driver’s license and acceptable driving record.
- Valid EMT (or higher) license, regardless of the state of issuance, at the time of application. At the time of job offer, a valid Oregon EMT license is required. Click here for Oregon Health Authority Reciprocity Requirements.
- Two (2) years of experience in a leadership or supervisory role.
The following minimum qualifications are required for this position: - Ability to interpret and apply statutes, regulations, policies, and procedures of privacy rules and laws, mandatory reporting, and legal responsibilities to clients and the community.
- Ability to observe a scene through a safety lens, recognize reactivity by clients, and effectively respond and mitigate these issues.
- Experience supervising and motivating a multicultural workforce and promoting an equitable workplace environment including training, scheduling, and performance evaluation.
- Ability to search, and navigate between different screens and mobile applications using a mobile data computer to document medical records.
- Ability to work effectively and collaboratively in multi/intra-disciplinary settings with other first responders and health and community agencies.
- Experience working with and communicating clearly, accurately, and completely to both internal and external stakeholders in a courteous, empathetic and professional manner.
- Knowledge of medical terminology and health systems navigation including an understanding of the principles and practices of Trauma Informed Care (TIC).
- Experience assessing risk of harm to self and or others, including suicide, homicide, and violence.
- Knowledge and understanding of cultural differences, especially as they relate to race, sexual orientation, gender- equality, socio-economic status and their intersections.
Preferred Qualifications The following are not required but are desired for this position.
- Degree/Certifications:
- Bachelor's degree from an accredited college or university in healthcare or related field
- Community Paramedicine certification
- Two (2) years EMT experience in emergency medical response.
